The gap between 'recommended' and 'audited for safe deployment' is where most of these systems fail. It's not a bug, it's just how the incentives are structured.

Was there a comparison to a note written the “old fashioned” way? Sadly there are often a litany of errors and omissions in regularly written notes. Still concerning though.
