Is Opus 5 really making really bad decisions? Is it possibly Claude Code in general? I’ve caught Fable and Opus 5 outright lying a lot lately and when it try pin down why it’s lying, it changes its story I can’t tell if something is off or I’m just noticing it more

Not making it up, probably: context drift. It committed to a decision earlier in the session, forgot it did, and now confabulates a story to cover the gap. Shorter sessions + explicit handoff state cut it down a lot for us.
