That’s funny, I used to work at Tyco (a.k.a. SimplexGrinnell, which has now merged with Johnson Controls). I inspected sprinkler heads for a year or two with the company and never ONCE heard anything about this. But knowing them it’s pretty much par for the course

The facility had not replaced dozens of recalled sprinkler heads, despite identification during a prior fire-safety inspection. The fire underscores the need to remind facilities of the dangers posed by unremediated sprinklers and encourage remediation wherever recalled units are still in use.

Infuriating that the problem was identified but not addressed until it was too late.
