I thought I could use one of those new-fangled AI tools to fin the deepest one in my code base but no dice, so I'm gonna go out on a limb and say 4.

You could probably just search for question marks in your code and come up with some heuristic for whether they're nested in an expression or not depending on the language.

Can all 4 branch (16? Cases) or should it generally be 1 chain of booleans and the other half is basically the default value case?

Single chains of booleans should really be the only legal use-case of "nested" terneries. Personally find them quite readable and use them several places in JSX "templates"
