That Pennsylvania figure stands out — is the spread concentrated in a few counties, or fairly broad across the 44? It would help to see how vaccination coverage compares in the hardest-hit areas.

Actually, no vaccine is 100% effective. The measles vaccine is either 93% or 97% depending on if one or two doses.
