I saw a uniquely bad one recently. I was looking up "fibromuscular dysplasia" on Google for class and the AI response found the abbreviation "FMD" somewhere online and then summarized an explanation of a different "FMD" ("foot and mouth disease") which had nothing to do with my search phrase.
