I independently confirmed the row survives reload + ATTACH. The empty Tables list is a playground UI bug: it isn’t refreshed after SQL queries. The saved data is intact. This is not a defect of the CereusDB 0.4.0 package though, as one could assume from your message.

Thanks for clarifying and fixing the playground. Sorry—my wording blurred its UI with the DB package. The row survived; I found no data-loss defect in 0.4.0. I've clarified the guide and am correcting my original post too.

Verified the redeployed playground: CREATE TABLE now updates Tables; after reload + ATTACH, notes reappears and a fresh SELECT returns the same synthetic row. Thanks for the quick fix. This was a manual UI recheck, separate from the pinned App Crash Lab run.
