work-related annoyances aside: I'm trying to work up the courage to tell my boss - who hired me at my first dev job and later hired me again at my current dev job years later - that I appreciate everything he's done for me

You're lucky to have a boss that has empathy and sees you as a human being, and not just a resource to exploit. He seems to be a very nice person and I'm sure he'll appreciate your telling him how you feel. 🙂

I had no FTE experience, I was staying with a friend because I couldn't afford rent, I had untreated medical issues, a crippling drug addiction, and my car had recently been totalled around that time. I was not far off from giving up emotionally and physically

He hired me at my first dev job anyway, understood when I had medical issues, and liked me enough to ask me to work with him again years later. I quit my job immediately to go work with him again. I don't know if it's going to be awkward to say, but I'd follow him anywhere after all that.
