cool study of my health code, a (non commercial) tablet-based #EndOfLife intervention for patients in the ER to get #GoalsOfCare conversations started & advanced #ACEP26

1 in 4 ED-initiate hospice & palliative care consults don’t end up being completed of those completed, <12h average time, much faster for hospice than palliative #acep26

small study of EM residents: report low amounts of palliative education, low confidence in skills, they WANT more palliative care education, and maybe a link with improved burnout #ACEP26

small qualitative adults with autism in the ED: communication and flexibility with environment (ie, move to quiet room) seem to work well to avoid escalation, sedation #ACEP26

prior cath (under 2 years ago) with non obstructive CAD was *not* sufficiently predictive of low risk chest pain… unless you include a negative hsTrop (which we usually do) #ACEP26

violent words, threats, and actions against ER health acre workers are both common and underreported lots of intoxication this study was among agitated patients so the numbers aren’t representative bit having observers record violence = shows underreporting @thesgem.bsky.social at #acep26

900k ED visits/y for asymptomatic elevated BP 0.7% adverse outcomes after DC which is not long term hypertension – may be reasonable to start BP meds this obs study showed 1 mo supply assoc w slightly better 30d CV outcomes @thesgem.bsky.social at #acep26

We have a telehealth protocol with the intent to keep asymptomatic and mildly symptomatic elevated BP patients who call triage from going to the ED, it’s very cool when it works lol

Unfortunately most of mine are older and maxed out on 3 BP meds managed by cards so I do more “urgent office appointment” scheduling than interventions at that juncture

Yeah we deal with folks checking BP at home or at the dentist office etc. So it’s a fight sometimes to keep them out of the ED, rather than them agreeing to go to a pcp appt in the next couple days, because the dentist or WebMD told them they’ll have a stroke soon etc. but sometimes it works!
