and this won't be a problem because every ethical doctor will just check a box that says "my 80 year old patient worked 80 hours this month"

The employer will be the one reporting hours worked, just like when someone is on parole. Goverment will copy other departments that do the same thing (tracking work hours for goverment oversite) instead of asking doctors.
