Can AI help personalize diabetes care? People living with diabetes often take multiple medications and need regular monitoring, but the information needed to personalize their treatment is often scattered and hard to find. A new DHIF project is exploring how AI can address this challenge.

CONNECT will build a “diabetes prescriptome”—a structured medication and care history linked to electronic health records and genomic data. AI models train on this to draft personalized clinical guidelines. For patient privacy, only the AI travels; patient records do not move between organizations.

Combining machine learning with causal modeling helps AI understand cause-and-effect patterns in the data and why certain treatments work better in certain patients. An AI platform supporting smarter clinical trial design could offer more personalized cancer treatment decisions. #datasharing

FLARE is designed with privacy at its core. AI models train where the data already live without moving patient records. Partners learn from data across institutions; patient information stays secure. FLARE will develop tools for timely, personalized patient support and reduce care team workloads.

A new project supported through the Digital Health Innovation Fund explores how AI can address these challenges. FLARE uses AI and federated learning to help healthcare teams ID patients at risk of missing doses, disengaging or leaving a trial, while also helping detect safety concerns earlier.

Based in Montreal, the project will create a scalable system for improving daily patient safety, where safety tools automatically flag risky dosing patterns in near real-time, protecting patients and also readying Canadian hospitals for next generation AI-driven #precisionmedicine research.

The lack of standard language across medical records makes it difficult for doctors to see the complete picture. By using international standards, the project team is making sure different systems can talk to each other, to transform siloed records into accessible for clinicians and researchers.

Using AI-augmented tools and real-time data this project will securely gather information from electronic medical records and clinical notes. AI tools will identify patterns that indicate worsening mental health to create models so that clinicians detect distress earlier and offer effective support.

The Ontario-based team members are creating a secure, intelligent system that helps clinicians detect distress sooner and supports better treatment decisions, connecting Canadians to timely, tailored mental health support through smart technology. #datasharing #AI

Precision medicine relies on analyzing big data for answers, often with AI to pull out patterns that the human eye cannot. But the answers it pulls out are only as good as the data that it trains on. With missing data, results generated can be biased and miss a whole section of the population.

The project will create a skin health network using data from across Canada, including from Indigenous people. Data will be de-identified for privacy & managed by provincial & Indigenous data sovereignty principles. AI-refined decision tools for clinicians enable equitable triage & faster diagnosis.

This patient-centred registry will be built to enable FAIR (Findable, Accessible, Interoperative, Reusable) data handling for privacy-preserving AI and machine learning analytics. Integrating patient voice and secure infrastructure ensures that innovation translates into improvements for patients.

Health data in Canada is often fragmented, making it difficult to research outcomes, treatment effectiveness and patient experience. This DHIF project is exploring how to combine patient-reported data with clinician and health system data in a strong governance framework to preserve patient privacy.

For additional security, the project partners will use federated learning, so that only the computational tools travel; the data stay safely under institutional governance. Insights could improve treatment decisions, accelerating #precisionmedicine research without compromising patient privacy.

The project will standardize hospital data to generate a synthetic dataset. Since this retains only clinical information and not personal details, it can be used to train AI tools and help analytical models learn, for valuable insight on treatment, diagnostic work ups, and patient outcomes.
