Some new patients now wait 6 to 10 months to see a primary care doctor. In my new @jama.com story, I dug into why. We're training too few PCPs in places that need them, making it harder for international doctors to stay, and underpaying the whole field. jamanetwork.com/journals/jam... #MedSky

What if you didn’t have to ban vaccines to make them harder to get? My latest @rollingstone.com investigation examines a potential “backdoor” strategy: changing the committees, compensation systems, and evidentiary standards that shape U.S. vaccine policy.

There are competing views about what these changes represent. Administration officials and allies say they are restoring parental choice, improving vaccine safety oversight, and ensuring that people injured by vaccines receive compensation. Critics see a fundamentally different agenda.

Experts I interviewed described a potential chain: Broader vaccine injuries → more potential claims → more litigation → greater uncertainty for manufacturers. The theory: you may not need to ban a vaccine to make it harder to produce or obtain.

The story also examines the possibility of recognizing conditions such as autism within the federal vaccine-injury compensation system—something that is not currently the case. That could have significant consequences.

The investigation also examines efforts to reshape the federal committees that advise on vaccines. Those efforts have encountered legal challenges. A federal judge wrote that one set of changes “undermined the integrity” of the committee by abandoning its technical expertise.

After those roadblocks, other mechanisms came into focus—including a proposal to change the committee’s charter and a new federal committee focused in part on autism. The stakes extend beyond the vaccine schedule itself.

At the center is the Vaccine Injury Compensation Program, created in 1986 to compensate people who establish certain vaccine-related injuries. A key question is what qualifies as an injury under the program. That’s where things get interesting.

The federal government maintains a “Vaccine Injury Table” specifying which injuries can qualify for compensation. Change that framework, and you potentially change the legal and financial landscape surrounding vaccines.
