Imagine being angry with the local tobacco shop because they “don’t care for their customers” when you buy a vape that loses charge too quickly. It’s a tobacco shop. They profit by selling you things that kill you. Why are you shocked that they don’t care for you?

Tomorrow, you borrow my ladder. You borrow my ladder and you give him the test. If he doesn't set up that ladder correctly for you, so you can climb safely - dump him. If he doesn't set up that ladder safely for you, it means he's a selfish man and you're only seeing the tip of the iceberg.

big rock big water 🪨🌊 (they/them)@bigrockbigwater.bsky.social

One time my ex set up a ladder for me and I climbed it and fell from the 5th rung because he just set the latches on the rung, not hooked them. I still have a scar on my leg, I was 16. Moral of the story, dump him before he dumps you off a ladder and you have to dump him 10 years later anyways 😂

Imagine it. 5:52 am. A physician mom tends to her son who woke with a fever, a headache, and a tummy ache two days after having licked the shopping cart at Sam’s Club. The dog stands nearby, barfing in the floor. It’s ok. I know you want my life.

If you are a specialist who wants to prescribe an NSAID for a patient with heart disease who takes Pradaxa, please don’t ask me as the PCP for approval to do that. I’m not here to be used as a shield for liability for you. We all make decisions to combine medications in ways involve some risk.

There’s no way around it. If you discuss that risk with the patient and they agree to it, I’m not going to judge you for it. Pain is difficult to manage in older patients with comorbidities. We have to make tough choices.

I went to get my teeth cleaned and my hygienist didn’t offer me a choice of paste flavors. I was assaulted with an aggressively mint rather than the cupcake flavor I usually choose. I am not ok.

What happens when you speak with a “clinician” in insurance appeals: “This medication is only FDA approved for children over the age of 6.” “She is 5 yrs, 11 mos, & 3 days. Do you think her metabolism will significantly change in that time?” “… I don’t know.” “Are you a physician?” “…………. No.”

Here’s the thing. These tech bro types keep saying that AI is going to change everything. But what is it going to do about the fact that patients can’t pay for the tests that AI says we should get or the meds that AI says they should take?

Bad Medical Takes@badmedicaltakes.bsky.social

AI doesn’t fix the fact that my patients don’t have a car that can make it far enough to get their colonoscopy. AI doesn’t fix that I have multiple adult patients who can’t read. AI doesn’t fix poverty or hold the check for a patient’s meds for a week until their paycheck gets deposited.

public health guy@publichealthguy1.bsky.social

rural health folks have been hearing nonsense and hype about how AI and medtech will fix everything for YEARS. if these coastal elites had bothered to talk to anyone instead of imposing their corrupt deals on everyone...

Patient’s insurance requires them to use CVS Caremark. I see patient last week. BP elevated. I send rx to increase valsartan from 160 mg to 320 mg. CVS Caremark sends me a notice that they’re out of refills for the 160 mg dose and that this may affect their compliance. Yes. I suppose it will. 🫠

I don’t know who needs to hear this (yes, I do) but please screen men for OSA before starting testosterone replacement. Untreated OSA is a contraindication to treatment. TYSVM for your attention to this matter.

If you’re going to take issue with the use of “y’all”, I’m going need to you stop using the word “don’t”. Both are contractures. It’s not “folksy”. It’s a legitimate word.

Minor point but I really hate this performative “y’all” people do to try and appear more folksy. I don’t type in a Yorkshire accent and I did people would rightly tell me to shut up.

Notice from patient’s insurance: this patient is late to refill their Rybelsus. Yes. Because that medication is no longer produced. And the replacement prescription for Ozempic tablets was filled three weeks before this notice was sent. The insurer paid for them. Super helpful.

I’ve never wanted to see it more.

BREAKING: Trump just ordered warning signs and exhibits to be installed outside the Smithsonian's National Museum of American History to alert the public that the museum does not, in his view, accurately reflect American history. For the record: It does.

Happy Wrensday Wednesday! Here's one from my archives. A superb fairywren portrait. This is the common type of fairywren where I live. #wrensday

Ok. The little pop-it things that pop loudly when you throw them on the ground… I’m the only one at this cookout who has ever snapped them with their fingers to make them pop. Tell me someone else here has done this.

Is there an excess of tragedy going on in the lives of people I know right now? Yes. Am I in the middle of it all because I’m the doctor and it’s my job to explain it all and help make plans and support them all? Yes. Is that completely overwhelming? Of course.

I love the Memphis medical community. I currently have multiple close family members facing similar awful medical diagnoses. One in Memphis. One in Nashville. Things in Nashville take FOREVER. In Memphis? We get a bad test result yesterday.

Today, we have three specialists in the OR together consulting on the same elective case because the primary surgeon called them after 6pm yesterday and this morning and they showed up, ready to get things done expeditiously. Memphis takes a lot of heat, but I love that city.

Hey I may not do your stupid modules, or publish enough, or close all those charts, or volunteer to teach at the medical school, but I always try to drop everything and go the extra mile for *actual* patient care

My dog got sprayed by a skunk last night. Despite having showered this morning, I smell like a skunk today since I had to bathe her. Embarrassed, I warned the tech doing my mammogram and she agreed. While I appreciate honesty in general, this is the one time when I’m begging you to lie to me.

Don’t like big pharma funding medical research? Great! Me either! But shut up about it if you’re not willing to vote for people who will publicly fund that research. We don’t make advances in healthcare with bake sales and raffles.
