New @cgdev.org piece w @rachelbonnifield.bsky.social on charting PEPFAR’s next chapter We propose a three-track framework, w some ideas on how to operationalize this evolution, with a healthy dose of caution on need for a prudent approach to safeguard PEPFAR's legacy

Rachel Bonnifield@rachelbonnifield.bsky.social

PEPFAR is at a great American success story at a crossroads. The Trump Administration's Global Health Strategy offers a broadly reasonable blueprint for PEPFAR's next chapter: more country ownership, deliverty by national governments, and spending cuts. 1/x

With major donors scaling back aid, the Global Fund’s $18B target for 2026–2028 looks out of reach. @jmadankeller.bsky.social & @clemencelanders.bsky.social propose a rethink of the Global Fund’s donor-dependent model. Learn more about the mixed grant & loan approach: https://go.cgdev.org/4odMAyJ

This is *not* a wholesale solution. It involves trade-offs & raises unresolved policy questions, but it offers many benefits: 1. It ⬇️dependence on donors 2. It provides a transition pathway🛣️for countries 3. It brings health money💰 on-budget 4. It stretches donors' contributions further ↗️

Drawing inspiration from elements of the World Bank’s model, we project a scenario where the Global Fund would continue to provide 💵 grants for low-income countries but would introduce loans 🏦 for middle-income countries

Amid widespread donor retrenchment, global health institutions, like the Global Fund, are facing an uncertain funding outlook. Given this new reality, we argue it's time to rethink the donor-dependent, grants-based model that the Global Fund has long relied on

Vaccines are one of the most powerful tools in public health. They have saved hundreds of millions of lives by preventing deadly diseases like measles, polio, and smallpox. Before vaccines, these infections were common; surviving them often meant facing severe complications or lifelong disability.

A week ago, the Trump administration laid off thousands of people at HHS and its agencies. The cuts were so deep, and touched so much, we're still learning about the impacts. Here are a couple stories that have stood out to me. 1) @levfacher.bsky.social on the gutting of a pain-research office.

statnews.comUnique pain research office eliminated in HHS purgePain is the most costly chronic health problem in the U.S. In latest RIFs at NIH, an entire division devoted to pain research was closed down.

The more detailed #PEPFAR limited waiver language is being sent to implementers. As has been noted, PrEP can only be offered to pregnant & breastfeeding women. BUT, it also says that even those already on #PrEP CANNOT continue on it. #foreignaidfreeze

Curious how many mothers and children worldwide will die from slashing USAID programs? We'll never know, because we're gonna stop counting.

One-third of all US global health assistance, just over $4 billion per year, has been directed through USAID in recent years. Shuttering its maternal, child health, tuberculosis, malaria, HIV and other programs will have predictable, terrible, results for those served. www.kff.org/interactive/...

Exactly this. Reviewing foreign aid for consistency w/admin policy goals is reasonable. Many programs can absorb a 90 day pause w/o catastrophic impact. But a real stop work on bi-partisan malaria & HIV programs--delivering lifesaving medicines & bednets--quite literally means people will die.

Charles Kenny@charlesjkenny.bsky.social

Glad to read there has been a waiver for emergency food aid, but the same has to urgently apply to other programs providing life-saving support where 90 days could be too late: PEPFAR, PMI... www.reuters.com/world/us/tru...
