How I am thinking about the news of a Russian lab researcher dying of an unknown pneumonia. 1. This is grabbing attention because it is a lab leak; the Russian govt appears to be suppressing info; and Russia might have an offensive bioweapons program which this could be related to.

cnn.comResearcher at Russian plague laboratory dies of โ€˜unknownโ€™ infection | CNNRussian authorities are responding to an incident in which a dangerous infection was released as a result of a laboratory accident in Siberia and a researcher died.

2. The big question is whether there is sustained human-to-human transmission. Is the infection jumping beyond the original infected patient? So far I don't see evidence that there is sustained transmission. I think it is OK not to worry until/unless there is transmission to others.

Iโ€™m a specialist doctor in Canada who opened a new practice. Several of my new patients are coming from AIโ€”patients are telling a chatbot their symptoms, and it gives them my clinic phone number. It feels very strange. Also, how long before AI companies start charging to recommend your business?

If you claim to have a high-risk condition, you do not need to specify what that condition is, nor do you have to present any proof. Your insurance should fully cover COVID and flu vaccines this year. I encourage you to get your updated COVID vaccine. [END THREAD]

The current federal government is intentionally making it harder for you to access vaccines. In general, if you are under age 65, to get a COVID vaccine you either need a prescription from a doctor or you need to attest to having a "high-risk condition."

FDA approves Moderna mRNA flu vaccine. For the same reasons that it was bad that the FDA initially refused to review the application, it is good that they approved it. www.nytimes.com/2026/08/05/h...

[THREAD] Why it matters that the FDA is refusing to review Moderna's application for an mRNA based influenza vaccine. 1. Flu is a perpetual threat to human health, because it sometimes mutates and sparks major, deadly pandemics (including the 1918 flu). Our best defense is vaccines.

Regarding the White Houseโ€™s announcement that Trump has a rash to his neck due to a โ€œpreventiveโ€œ skin cream: -this is almost certainly fluorouracil (probably with calcipotriene added), which treats certain pre-cancers (actinic keratoses) and early stage squamous cell carcinoma. Not deadly.

5. The hostile regulatory and trade environment will drive vaccine manufacturers out of the US. Vaccines will be made in other countries. There's an advantage to having robust vaccine manufacturing in your country--you get dibs when there's a pandemic. The US is going to lose that advantage.

4. An advantage of mRNA vaccines is that an mRNA manufacturing facility can make many different vaccines, and switch between them. That lets you surge production of any particular vaccine if there's an outbreak. You can only use eggs to make vaccines for flu and MMR, and they can't be scaled up.

The beauty of an mRNA influenza vaccine, such as what Moderna has developed, is that you can manufacture it extremely quickly, and you can easily and quickly adjust the vaccine strain to match the virus, even as it mutates. Eggs can be finicky, and might not grow the exact vaccine strain you want.

2. The decision shows that FDA can't be trusted, because the FDA spelled out for Moderna exactly what it wanted in a drug submission. Moderna followed those instructions, and the FDA rejected the application anyhow without reviewing it. Goodbye US drug development.

But our current flu vaccines are not ideal. One major issue is, even in a crisis, it takes many months to make a vaccine against a new flu strain. For one thing, flu vaccines are still grown in chicken eggs, and it takes a while to grow enough vaccine in enough eggs to cover the population.

Also, you don't want a vaccine supply that depends on chicken eggs, because if there is a flu that infects both humans and birds, and it kills the birds, now you might lose the things that lay the eggs that make your vaccines. (Chickens that make vaccine eggs are sequestered to prevent this).

A legend of public health has died. Dr. William Foege co-led the successful global effort to eradicate smallpox, and later was director of the CDC. I quite recommend his book, "House on Fire." It is a remarkable account of smallpox eradication, one of the greatest public health achievements ever.

Also, IVs can be inserted plenty of places that aren't people's hands. I just don't think the bruising to the hands is a probable sign of some unexplained medical condition. I am not saying Trump is healthy. I am not saying Trump should withhold his medical records...

I am saying that the bruising to the back of is hands can be explained very easily just with the information we already have. I think itโ€™s a red herring. Below is link to article where White House says heโ€™s on aspirin.

Solar purpura is bruising to the backs of the hands due to prior sun damage, old age, and sometimes anticoagulants including aspirin. The chronic sun damage makes the skin and superficial blood vessels very fragile and prone to bruising and bleeding.

We know that Trump is old, has fair skin, spends a lot of time outside golfing, and has visible sun damage to his face. The White House press secretary has said he is on aspirin. And he shakes lots of people's hands, which triggers bruises. It would be surprising if he didn't have solar purpura.

[THREAD] The federal government is trying to hinder gender-affirming care. Something interesting from a medical standpoint is that gender and sex are so complicated, that I don't think you can interfere with care for transgender children without also messing with other children's care.

Under the proposed rule issued by the Trump Administration today, any Medicare- or Medicaid-participating hospital would be barred from providing gender-affirming care to *any child*, even if the care is not paid for by Medicare or Medicaid. public-inspection.federalregister.gov/2025-23465.pdf

I don't think it's possible to regulate transgender care without disrupting care to these other patient groups. It's yet another reason why attempts to regulate gender-affirming care are problematic. When it comes to gender and sex, the government should leave doctors and patients alone.

There is a condition called androgen insensitivity syndrome, where the body cannot respond to male sex hormones and therefore the body develops as female. These patients' sex is technically male but they are essentially female. Does the federal govt want us to treat these patients as male?

But it gets even more complicated. There are patients who are only partially responsive to male sex hormones (for example, the Gรผevedoces in Dominican Republic). They have female genitalia as children and male genitalia after puberty. How to treat those patients under the federal guidelines?

-That doesn't mean vaccines are safe. The FDA is signalling that it will try to regulate vaccines into oblivion. RFK can force vaccine manufacturers out of business by saddling them with massive liability whenever a patient accuses them of a vaccine-induced injury.

-ACIP is just one front in the administration's attack on vaccines. I don't even think it's the main front, because its members seem to be worried about angering the public with their anti-science/anti-vaccine machinations.

Remember, this is a multi-front assault on vaccines. -Suspending funding for vaccine research -Hindering vaccine approvals -Hindering vaccine updates -Reducing vaccine access -Altering vaccine recommendations -Shifting costs to patient -Spewing antivaccine misinformation -Sowing general confusion

-My experience in federal government is it's much easier to slow things down than it is to reverse things. It's much easier to slow the approval of new flu vaccines than it will be to take flu vaccines off of the recommended immunization schedule. It's easy to subject things to additional reviews.

-Why did ACIP go after Hep B vaccine first? Perhaps it's because the consequences are so far in the future. If newborns acquire Hep B now, they may not find out until decades later when their livers fail. If you go after MMR or flu or COVID vaccine, people get sick right away.

-ACIP could have passed a resolution that would cut off federal subsidies (via Vaccines for Children) of Hep B vaccines, but they didn't. I think all the ACIP members understand, deep down, that vaccines work. And they're scared of blowback if they fully cut off access to childhood vaccines.
