Truth. The advances in medicine alone are astonishing.

My unpopular but persistent belief is that almost everything in our lives has gotten better and we’re weirdly incapable of seeing it.

This is a good time to remind folks interested in #CBME and #GME to provide feedback to the @acgme.bsky.social about their proposed revision to the Common Program Requirements for residency training. Comments close today! www.acgme.org/programs-and...

acgme.orgReview and CommentThe ACGME invites comments from the community of interest regarding the proposed Requirements listed below.

We are delighted to have had @ewidera.bsky.social join @nathanielchinmd.bsky.social on his Dementia Matters podcast, discussing functional assessment and blood-based biomarkers of #Alzheimers disease in primary care. #medsky

Though blood tests have made it easier to diagnose Alzheimer’s, they can’t replace the diagnostic process as a whole. Dr. Eric Widera joins #DementiaMatters to discuss the importance of functional assessments for dementia and how blood tests should be used in primary care. Listen🎧 go.wisc.edu/p8td3q

Great write-up about the work of @uwmadison.bsky.social School of Medicine & Public Health colleague, Dr. Daifeng Wang, who is using informatics approaches to better understand #Alzheimers disease. #dementia #medsky

1- Congratulations to Dr. Karl Deisseroth, Department of Psychiatry and Behavioral Sciences in the Stanford School of Medicine, on winning the Nobel prize! Dr. Deisseroth is the third psychiatrist to win a Nobel Prize. #medsky

Howard Liu, MD, MBA@drhowardliu.bsky.social

“The Nobel Prize in medicine was awarded Monday to three scientists working on the mechanisms the brain uses to switch on, or off, the activity of individual nerve cells.” www.npr.org/2026/10/05/g...

2- The first two psychiatrists to win Nobel prizes were Julius Warner-Jauregg (treatment of neurosyphilis via malaria-induced fever) and Eric Kandel (basic science of memory).

Bipolar disorder has among the highest mortality rates of any psychiatric disorder (suicide, medical comorbidities). It turns out that increased mortality persists into late life, according to the results of this Dutch cohort study. www.ajgponline.org/article/S106... (sorry, paywall) #medsky

Antidepressants are effective treatments of major depression & anxiety disorders. US veterans have ⬆️ risk of depression, anxiety & suicide than others. The VA’s new requirement for informed consent for antidepressants could be barrier to effective care. www.nytimes.com/2026/09/28/s... #medsky

Great summary of the finding that monoclonal antibody treatment of Alzheimer's disease can result in whole brain volume loss ("pseudo-atrophy"), though no change in hippocampal volume. The magnitude of volume loss correlates with the incidence of ARIA.

Dr. Fick shared this terrific fishbone diagram listing causes of undetected #delirium in healthcare settings. She also noted @amaclullich.bsky.social's seminal work in delirium, including the development of 4AT. #medsky

3- In "Paradiso," Dante encounters Beatrice, a woman he loved who had died. This contact transforms him: "There is a continuing bond - his eyes intent on quenching 10 years of thirst at last - yet he has formed a new relationship with her; he is seeing her as he has never seen her before."

4- The author of this piece argues for an empathetic and nonjudgmental approach for physicians whose patients report after death communication, helping them "understand and process these experiences," resulting in "increased sense of meaning in life, and decreased anxiety toward death."

2- Reports of after death communications are common in older adults: 73% report feeling as though a dead loved one was looking over 49% as though a dead loved one was in the same room 24% hearing the voice of a dead loved one 21% seeing a dead loved one

3- He cites #POTS as an example: "Treating POTS requires addressing the whole system. You are not fixing a broken part. You are helping a dysregulated system find new equilibrium."

4- Regarding the relationship between physicians and patients, Grubb argues for a restoration of "restores what reductionism has squeezed out: the recognition that healer and patient meet not merely as technician and mechanism, but as two expressions of the same underlying reality."

2- Since "complex biological systems display emergent properties that reductionism cannot predict or explain," a reductionist paradigm of breaking the body into parts & fixing what is broken is incomplete.
