Think of it as an mCODE for Europe, aligned with mCODE/CodeX and OHDSI Oncology and built on the HL7 Europe base profiles behind the EHDS exchange format. Collect once, use many times: from the EHR and tumor board to registries, trials and real-world evidence.

Germany's AWMF is moving its register of 850+ guidelines, mostly PDFs, onto FHIR through Dissolve-E. But digitized is not yet computable. Structured GRADE and PICO on FHIR is not logic that runs against EHR data.

The Toews et al. scoping review (122 studies) shows the same issues everywhere: ambiguous source text, conflicting guidelines, terminology mapping, IT integration. And a clear trend toward FHIR. EBM-on-FHIR plus CPG-on-FHIR could cover the full evidence chain, up to living guidelines.

I didn't expect a call for computable guidelines in a US Senate bill. SCREENS Act (S.5520, Grassley/Hassan): ONC would publish cancer screening recommendations as computable logic. Within 5 years, certified EHRs must run it to determine screening status.

It names #mCODE and #USCDI, not #FHIR or #CQL. But hose are the natural fit. If the bill passes and ONC picks open standards, every certified US EHR gets an engine for shareable guideline logic. That is the infrastructure computable guidelines have been missing. www.linkedin.com/pulse/new-us...

#CQL is an open language for clinical logic. Quality measures. Decision support. Computable guidelines. Cohorts. Readable for authors, executable for systems. Older than the #FHIR boom. Not married to FHIR either. But most energy is on CQL with FHIR now, via big quality measurement efforts.
