Our latest video in partnership with @glaucomflecken.bsky.social summarizes the ALLEGORY trial that evaluated the efficacy and safety of obinutuzumab when added to standard therapy in adults with active systemic lupus erythematosus. Read the full study for free: https://nej.md/DrG49 #rheumatology

There’s a stray cat who comes around our house. He was looking skinny, so we’ve been leaving out cat food. Haven’t seen him in awhile, but the food was gone every morning, so I assumed he was eating it. Turns out a fat ass possum has been eating it. Asshole has been living like a king on cat food.

one time i took my broken glasses to an old school eyeglass repair shop and the guy did a fantastic job, couldn’t even see the original break, but i saw a bit of smudge on the lens so i tried to wipe it with my t-shirt and he yelled “no!” at me and snatched them back to clean them properly

People do this in healthcare and it always irritates me. “Look at that patient. They sit there with an iPhone but say they can’t afford their medications?” Buddy, you are mad at the wrong thing.

Qasim Rashid, Esq.@qasimrashid.com

Never take seriously those who ridicule low income people for having a smartphone or laptop. “If they’re so poor how did they buy an iPhone???” Because they’re trying to escape poverty, not the 1700s, genius. If you think basic tech access is unnecessary or a luxury, survive a month without it.

I’m fascinated by scientific breakthroughs that were universally hailed in the moment, like pasteurization, that are now vilified by some people. Is this a uniquely American thing? Anybody written a book about why/how this happens? Like the psychology behind it. I’d love to learn more.

I’m curious why Casey Means actually quit in *year 5* of ENT residency. Like I get burning out and leaving medicine to own a bakery or something, but the stated reason is bc she wanted to treat the “root cause” of disease? What? ENT gotta be top 10 in specialties that cure disease. It’s surgery.

This is part of what makes health care feel so hopeless. We should have more power as consumers, but where do you go if you don’t like your health insurance company? Just die, I guess?

This is what #TeslaTakedown is about, to me: fascists have ignored all of the nice, polite, norms of how we are supposed to wield power in politics, which means those of us who oppose them must find and embrace other means of wielding power as well. Our power as consumers is massive.

I posted this last night as a joke, but after sleeping on it, I’m even more convinced it’s a genuinely good idea.

Bring back the stocks. Bad consult for some stupid kidney thing? Spend an hour in the stocks. Public humiliation is underutilized at the attending level.

Each specialty gets 3 things that are “stocks worthy” For ophtho: 1. Fail to remove contacts from intubated patients 2. Any consult without a vision assessment 3. Ophthalmology consult for blurry vision which I cure by putting the patient’s glasses on their face Trust me this will correct behavior

I still check in on the bad place & post there occasionally. I’m used to people shitting on docs, but these MAHA appts have brought out the worst of humanity. I honestly don’t think it’s safe for physicians over there. These are people who find your address and report you to the medical board

And the thing is, in the US healthcare system, physicians (and everybody who sees patients) are the ONLY people who actually got into this business to help patients. Every other aspect of US Healthcare is designed to milk your body for as much money as possible without care for you as a person.

Of course there are some awful doctors out there, but man, there is still at least a small piece of all practicing docs who got into this for the right reasons, & for most of us, it’s a big part, so it really fucking sucks to see people gleefully reject that in favor of wellness influencers and RFKJ

Amazing. UHC shareholders are suing because after the CEO shooting, the company was “no longer willing” to use “aggressive, anti-consumer tactics” to generate the profits they were promised. A gift from the content gods.

Fortunately, I have an eye drop called Night Sight ™️. Two drops per day throughout your child’s life will not only restore night vision, but supercharge the photoreceptor opsin proteins, expanding their electromagnetic spectrum into wavelengths not normally seen in human vision. New Colors!

I could absolutely make a million bucks off this idea. It is so easy to grift when you have letters after your name. Previously, these people were relegated to the fringes of society. Now, they become US Surgeon General.

Is pseudoscience cyclical? Like will the whole metabolic/seed oil/diet-cures-all-disease stuff eventually burn out allowing for a return to critical thinking, or is this just it now. Did this all happen 40 years ago and we just need to ride it out? Looking for a bit of hope.

Lmao it was all a fucking act. I mean, we all knew it, but it’s funny to see his hypocrisy laid out so beautifully.

Exclusive: In his first address to FDA staff on Wednesday, new CBER director Vinay Prasad emphasized his commitment to evidence, his admiration for agency employees, and his desire to serve the American public. Read more, based on a recording obtained by STAT: www.statnews.com/2025/05/07/v...

Heard a great talk by a commercial airline pilot about safety regulations for pilots compared to surgeons. I asked if older pilots get mad when younger pilots voice safety concerns because they think the only way to be a great pilot is to work while exhausted and he looked at me like I was crazy.

My 13 yo daughter was on the computer laughing. I asked what was so funny. She said “I found this website called The Onion it’s so funny” and reader lemme tell you, the elder millennial dad noise I let out could be heard 3 towns over.

“what do you understand about your condition?” “only what they’ve told me” “what have they told you?” “it’s in my chart” “can you summarize it for me?” “doc, i’ve been a patient since before you were born. i know how to evade any question to conceal my cognitive decline. i can do this all day”

I understand this concern, but the anti-vaxx arguments are already everywhere on social media. We have to have people refuting this stuff. Can’t let it go completely unchecked.

Jeffrey Wuhl@jeffreywuhl.bsky.social

Do you worry that he’s giving them credibility by “debating” them?

I don’t think it’s possible to change minds on vaccines in that format (maybe any format), but Dr. Mike tried and he represented the medical community well by staying level headed and compassionate. I applaud him for that.

Ray@razor823.bsky.social

My favorite moment is when he asked the one person, “Is there anything I could say that would make you change your mind?” and she just said no.
