🔥 Brilliants minds debate oral fosfomycin for complicated UTI including pyelonephritis. With increasing ESBLs, we need options for UTI beyond cystitis. Oral fosfomycin is an appealing option, but is it ok for infection outside the bladder? #pharmacist #pharmacyschool #learnantibiotics #fosfomycin

The next Clarity commentary. On the SNAP trial. Neither I nor @gurujosh.bsky.social had anything to do with this commentary (deliberately). Conclusions though are in line with my interpretation of the data. i.e., use cefazolin for MSSA bacteremia. @angelahuttner.bsky.social @cmicomms.bsky.social

In the SNAP trial, cefazolin was noninferior to flucloxacillin or cloxacillin with respect to 90-day mortality in patients with methicillin-susceptible Staphylococcus aureus bacteremia and had fewer nephrotoxic effects. Read the full trial results: https://nej.md/4vL0M6m

Great post @apsmunro.bsky.social! Doing more is such a natural reaction. But even 'benign' interventions may likely be harmful when tested properly. I'm yet to know of a trial for sepsis where adding antibiotics has been beneficial (outside of TB where we do it to prevent emergence of resistance).

Alasdair Munro@apsmunro.bsky.social

My new post looks at two antimicrobials which have been routinely used and even recommended in guidelines despite a lack of RCT evidence In the last few weeks they’ve both been shown to have no benefit, and worse, high probability of harm - killing patients A lesson we keep failing to learn

This is a really helpful graphic summary of the Andes virus outbreak. #IDSky

Version 9. New Canadian case (seen one link saying moved to hospital 14th, so presumably onset date), confirmed 14th. Fixed "2">>"3" April for Case 1. We're past the 42nd day - 6-week - of incubation since Case 1 onset. Can be as long as 8-weeks in a small percentage. Still all boat-related

ESCMID 2026 highlights Dalbavancin PK (DOTS‑PK) – Two doses may not be enough High protein binding and wide inter‑patient variability: 1/3 patients may be sub‑therapeutic by day 22 Bottom line: Consider monitoring levels or adding a third dose in complex infections clarityinitiative.org/home

So good. Have spent years fighting this fight, with hand tied behind my back by lack of great evidence. This study is going to get quoted and cited in microbiology MDTs the world over +++ #IDSky #ClinMicro #AMR

ESCMID 2026 highlights COBRA Trial – One day is enough Dutch RCT: 1 day of antibiotics after ERCP drainage for cholangitis was non‑inferior to 4–7 days. Bottom line: Antibiotic durations can safely be much shorter once source control is achieved. #IDSky clarityinitiative.org/escmid-2026-...

Just woke up and checked emails to see that 8 new participants recruited to #SNAP_Trial in the past 8 hours! While a number of domains have closed (PSSA, MSSA, adjunctive clindamycin) we continue to recruit to other domains. Beyond 5,800 total participants now. @gurujosh.bsky.social

Medical research spending has a positive return on investment; for every $ invested there's an estimated $4 return. Beyond economics, delays hold up medical innovations Australians deserve better If you agree, please sign the petition to let #auspol leaders know why medical research matters #NHMRC

Right now, the Government is starving medical research of full funding, even as it claims that science and research are national priorities. If our leaders truly value science as much as they say, then they must back their words with action—and fund it properly. Join the campaign!
