Study in 8 hospitals with 40,021 pts cut follow-up blood cultures by 18% (goal: 10%) ⬇️ without ↑ stay, mortality, or readmission. Diagnostic stewardship worked.🩸📉

cambridge.orgA quality improvement project to reduce follow-up blood culture utilization through diagnostic stewardshipView abstract Objective:To reduce the use of follow-up blood cultures (FUBC), that is repeat blood cultures (BCs) used to monitor patients for clearance of a prior positive BC, or with new/worsening symptoms after negative BCs, by 10%.Design:Multicenter quality improvement study.Setting:A network of 8 hospitals, including academic tertiary care centers and community hospitals.Participants:40,021 patients, including adult, pediatric, inpatients, and outpatients, for whom blood cultures were ordered over a 24-week period in 2024.Interventions:We implemented a series of iterative Plan-Do-Study-Act (PDSA) cycles to guide clinicians toward appropriate BC ordering. These cycles included educational memos, limiting the ability to order more than one BC at a time, a best practice advisory in the electronic health record to limit BC orders with a prior negative within 72 hours, and a new order panel for BCs that defaulted FUBC to a single set. Results:Prior to the intervention period, FUBCs comprised 29% of all BC orders placed. After the intervention, there was an 18% absolute reduction in FUBC, primarily due to decreased use of paired FUBC sets. There were no increases in the length of stay, in-hospital mortality, or 30-day readmission. Conclusions:We exceeded our goal to decrease FUBCs using diagnostic stewardship. This approach to FUBCs is sustainable and scalable as part of diagnostic stewardship strategies for BC stewardship.

Cefepime–zidebactam showed 91.7% ✅ clinical cure in 60 pts with CR GNB (82.4% MBL producers). 28-day mortality 2.8%⚰️. AEs in 15.3%, serious 2.8%. Well tolerated👍.

academic.oup.comCefepime–Zidebactam for Infections Caused by Carbapenem-Resistant Gram-Negative Bacteria, Including Metallo-β-Lactamase Producers: A Phase 2, Open-Label, Single-Arm, Multicenter StudyInfections caused by carbapenem-resistant (CR) Gram-negative bacteria, particularly metallo-β-lactamase (MBL)-producers, have limited treatment options. Cefepime–zidebactam retains activity against these pathogens, but clinical data are limited.MethodsThis prospective, open-label, single-arm, multicenter phase 2 trial enrolled adults with CR Gram-negative infections. Participants received intravenous cefepime–zidebactam for 7–21 days with renal dose adjustment. The Micro-ITT population included participants with a confirmed qualifying CR Gram-negative pathogen, and the CME population included those meeting prespecified evaluability criteria. The primary endpoint was investigator-assessed clinical cure at TOC in the CME population. Outcomes were also reviewed by an external adjudication committee.ResultsSeventy-two participants received treatment; 68 were included in the Micro-ITT population and 60 in the CME population. Among 68 qualifying isolates, 56 (82.4%) were MBL producers, predominantly NDM-β-lactamase producers. Investigator-assessed clinical cure at test-of-cure in the CME population was 91.7% (95% confidence interval, 81.6%–97.2%), and favorable microbiological outcome at TOC was 90.0%. Among participants in the CME population with MBL-producing pathogens, clinical cure was 93.9% and favorable microbiological outcome was 91.8%. External adjudication in the CME population yielded clinical cure of 86.7% and favorable microbiological outcome of 85.0%. Twenty-eight-day mortality was 2.8%. Treatment-related adverse events occurred in 15.3%, and serious treatment-related adverse events occurred in 2.8%.ConclusionsIn 60 evaluable adults with carbapenem-resistant Gram-negative infections, predominantly MBL-producing (81.7%) and cUTI/AP (58.3%), investigator-assessed TOC clinical cure was 91.7%; post hoc adjudicated cure was 86.7%. Day 28 all-cause mortality was 2.8%, and cefepime zidebactam was well tolerated.

ID is the best specialty; a pet raccoon knocks over a bottle of aromatherapy contaminated by Burkholderia and dies; the exhumed raccoon indeed tests positive for the organism (an emerging pathogen in the US)

100% these tests should be restricted to ID and perhaps hem onc in setting of a protocol (lung nodules in febrile neutropenia for example)

Primary endpoint : 42 d ACM 23/8% in olorofim arm vs 24.3% in SOC arm - non inferiority was met! Secondary endpoint: survival over time and day 84 all cause mortality - no difference The outcomes were similar to other large studies in IA recently Blinded MSGERC-EORTC response also same between arms

The clinical question: what to do when you have aspergillosis in a patience in whom you can't treat with first line treatment (a mould active azole). Current SOC is liposomal amphotericin B L-AMB) Olorofim is a dihidroorotate dehydrogenase inhibitor with activity against aspergillus & other moulds
