Thus, to experience pain properly, the brain requires a well established, well balanced network architecture in which bottom-up input and top-down context are properly integrated. An imbalance in network level processes may also have connotations for abnormalities in how pain is processed

🚨 8 months after, this paper found its home at Science Advances. We came up with a generative brain network model based on game theory and found that the human brain is suboptimal both in terms of wiring cost and communication efficiency. Here's why: www.science.org/doi/10.1126/...

Lim et al. find that children experiencing pain and exposed to adverse life events showed accelerated brain maturation in emotion- and pain-associated regions, demonstrating additive effects on brain development. Learn more in #PAIN bit.ly/3SnUAmF

1/ Chronic pain and post-traumatic stress symptoms (PTSS) co-occur very commonly, yet how they interact in the brain is unclear. New from our lab: trauma symptoms reorganize working memory circuits in chronic pain, even when behavior looks normal. Veinot & Hashmi, PAIN, 2026 🧵

8/ The takeaway: even trauma symptoms below the clinical threshold seem to reshape how prefrontal and brainstem circuits work together in chronic pain, and they do it through the emotional side of pain, not the physical. Maybe we should stop treating trauma and pain as separate problems.

6/ Behavioral relevance: these patterns tracked depression and pain catastrophizing, but not pain intensity or disability. PTSS appears to act on the affective dimension of pain, suggesting changed affective regulation.

7/ A subregion dissociation sharpened it further: dlPFC–dl/lPAG connectivity tracked arousal/reactivity symptoms; dlPFC–vlPAG tracked avoidance. This maps onto known active vs. passive coping roles of these PAG columns.

4/ Task connectivity showed complementary effects. Higher PTSS scaled with stronger dlPFC–periaqueductal gray (PAG) coupling during task. Crucially, this was specific to PAG. No effect for amygdala or hippocampus, pointing to descending modulatory circuitry rather than general affective processing.

5/ And connectivity and activations were linked: greater dlPFC–PAG connectivity predicted lower dlPFC recruitment at high load. Indicates prefrontal disengagement coupled to threat-circuit engagement under cognitive demand.

2/ 53 chronic low back pain participants did an N-back task during fMRI across four loads (0- to 3-back). We tested PTSS both dimensionally (PCL-5) and as a high/low split, and asked whether trauma load shapes task-evoked activation and connectivity.

3/ Under the highest load (3-back), higher PTSS predicted reduced activation in left dlPFC and left IFGpo, core lateral prefrontal control regions. The effect was load-dependent: absent at low demand, emerging only when executive demand peaked.

CIHR has launched a trial ballon that suggests shifting to one competition a year. This is the worst idea I have heard in a long time. The only benefit is making top-line success rates look better than they are. Keep it as is, do in-person reviews, and keep the pain in the open.

Key takeaways: -Chronic pain lacks objective biomarkers, making AI predictions especially vulnerable to misinterpretation -Expert-driven feature selection improves both model performance AND fairness -Explainable AI is essential; black-box systems undermine clinical trust and patient autonomy

-Transdisciplinary teams are a necessity, not a nicety, for equitable pain care -AI can help us move toward personalized, scalable pain care, but needs to be built with human oversight to be most effective
