In this systematic review & meta-analysis, Mortazavi & colleagues (@petermacp.bsky.social) investigate urban vs rural differences in #TB prevalence between 2000 and 2024 in 26 low- and middle-income countries, finding that TB epidemics have become increasingly urbanized 🧪 #MedSkyplos.io/3O79BqY
#TB is often portrayed as a disease of working age adults, but our new preprint shows prevalence of untreated disease is highest among older adults, who also face the greatest gaps in detection and reporting.
www.medrxiv.org/content/10.6...
We have a new preprint out:
In metaanalysis of 82 #TB surveys (30 countries, 4.6M people), we investigated how prevalence changes with age
Untreated TB increases substantially with age, with poorer case detection in older adults.

We have two major papers out, showing:
1) A new way to measure #TB at local level with convenience sampling & IGRA testing journals.plos.org/globalpublic...
2) Targeted case finding using this approach identifies more TB, reduce DALYs & improve cost-effectiveness journals.plos.org/plosmedicine...
As TB epidemics concentrate in vulnerable populations and area, we think this is an important new way to measure recent transmission, and target and measure intervention effects.
We have now expanded this approach to Lusaka 🇿🇲 and Cape Town 🇿🇦 through a @wellcometrust.bsky.social discovery award.

We then constructed mathematical models using these data to investigate the impact of:
1. Passive case finding
2. Untargeted community screening
3. Local ARTI-guided screening
Health outcomes (life expectancy, DALYs) and costs were investigated using microsimulation.
Compared to PCF, untargeted ACF improved life expectancy by ~3.3 years for people with #TB
ARTI-targeted ACF covering 50% of the population identified 80% of people with TB and achieved a lower cost per DALY averted

Firstly, we recruited 1545 children 1-4y using clinic convenience & random household sampling in Blantyre 🇲🇼
6.6% had MTB immunoreactivity, giving an annual risk of TB infection of 2.7%. Risk was highest in the poorest neighbourhoods.
There was substantial variation between local neighbourhoods.
Local ARTI was not correlated with case notification rates, indicating barriers to diagnosis.
Overall, Mtb immunoreactivity surveillance is a new approach that can help model local TB transmission, identify high-risk areas for intervention, and find TB-exposed children who may benefit from TPT.
Very pleased to contribute to this meta-analysis.
Among contacts of #TB, protection from preventive therapy waned quickly in high incidence settings, but persisted for years in low and medium incidence settings

At @who.int request, @petermacp.bsky.social and a multinational team led an analysis of cause and risk of death for people living with #HIV admitted to hospital.
1 in 6 people with HIV who were hospitalized, died.
In-hospital survival hasn't improved in the last 20 years.

Overall mortality remains unacceptably high (16%) & probably hasn’t changed since 2000

It's been 10 years since we published our first review of sex differences in #TB prevalence. In this update, we found men remain twice as likely as women to have TB, and disparities have likely widened over time.

We updated our review of sex diff. in #TB prevalence
In 102 surveys (4.7M participants) men were 2x as likely to have TB than women
Evidence consistent with sex differences widening over time

TB is preventable and curable - yet millions are still affected every year.
The problem isn’t just medical. It’s social, structural, and gendered.
This video shows how LIGHT is working with communities to change that.

Fantastic video from @lightontb.bsky.social PhD student Dr Mphatso Phiri, summarising the research he has been leading in Malawi 🇲🇼 to reduce men’s risk of #TB, and benefit men, women and children.
Why are men more affected by TB in Malawi?
Higher exposure starts in adolescence-linked to smoking, alcohol use & high-risk environments.
Good news:mobile, community TB testing is reaching more men & connecting them to care.
Tackling TB in men protects whole communities:
