STATS NERD TAILORING HIS FORMULA TO COVER THE SPREAD: The dew point at ground level isn’t conducive to deep sinkers. Pitchers are disadvantaged when throwing north PITCHER WITH THE CLUBHOUSE NICKNAME “BIG WHITEY”: if I don’t shit before the game I bet I could huck this straight through a guy’s head

One of the most banally evil things i regularly interact with on a daily basis is the way that medications with established clinical use in a disabling, fatal condition are rationed or withheld from patients who do not fit a narrow criteria of what they perceive as the only accepted way to get it

Which is, frankly, so beyond evil that it feels like a joke. Even in the world where this is just barely cutting the mustard as far as slowing rate of functional decline, that's still *something*. These people are staring down the barrel of a median survival rate of 2-2.5 years, <3% living 10 years.

What often happens with drugs like this is that following approval, the insurance companies review the Phase 3 (randomized clinical trial in affected patients to compare its efficacy vs placebo) study and then set up their clinical criteria for coverage to follow it. The study becomes universalized

So you have patients who have received a terminal diagnosis, often months after they get a name to their sudden, rapid decline, and their doctor says 'there are two medications that will provide you some time to live your life as it is' and then your insurance goes 'nah, too late, buster'

Radicava has a billed cost (what you send to insurance, not what they or you pay) of $15,000 a month, but it slows functional degradation in early-stage ALS patients by a mean of 2.5 points (ALS is assessed by a scoring scale of functional activities, the ALSFRS-R, each rated from 1-4, with 1 worse)

It was approved for a very narrow segment of patients - ALS patients who have been diagnosed, have a 2 or greater in *all* scores on the ALSFRS-R, and a FVC (Forced Vital Capacity, a measure of lung function - scored by a percentage of the expected total volume of your lungs - lower is worse) of 80%

ALS, Amyotrophic Lateral Sclerosis (or Lou Gehrig's Disease as it was once known), is a motor neuron disorder that results in progressive and fatal loss of control of upper and lower motor neurons, eventually leading to being unable to speak, eat, drink or breathe without mechanical assistance

Treatment is supportive - physical therapy, speech therapy, nutritional support and One generally available drug (Riluzole, approved since 1995) that has existed since 1995 that typically improves prognosis by about 3-6 months. In 2017, Radicava was approved for use in the US for early-onset ALS

I know realistically tatami is probably a pain to maintain and live with but every japanese home reno video i see from a youtuber i like who does carpentry makes me go 'no...don't...you can change her...it doesnt have to be this way'

Hey everybody, check out my new longform fic concept. let me know what you think!

Drinking one coffee stout (5.6% ABV) on an SNRI and forgetting to drink water afterward is a bad idea. Doing so after cleaning all morning that day and disturbing all the dust in your bedroom is what i would describe as A Bad Idea

I’m digging that Drifter Deadlock is actively ruining some young people’s vocal cadence in the same way Gambit from X-Men did my generation.

I know everyone is going to kill me with hammers for this one but I hate the period before daylight savings kicks in in the fall where the sun isn't up till like 8. I get up at 6:30 and nothing feels worse than waking up at o'dark thirty and having to get up anyway

Why do they call it the World Series when only Man (人) and Earth (地) are represented by the teams in the Brewers (humanity's greatest invention, fermenting) and the Rays (creatures of the vast ocean)? They totally forgot to include Heaven (天)...

Proposing that a third team be entered into the World Series and every inning the Mandate of Heaven (天命) is passed between teams based on portents and field circumstances, and that team's roster is added to that team's own during that inning. If the team that wins has the 天命 at the end, they win too

Actually it should be the All-Star team now that i think about it. worth more to the team with fewer all star representatives because it means they can take away someone's good players if they get the 天命 at the right point, but you run the risk of them not integrating well with the team...

Sometimes i watch a DS9 episode and its like 'Quark's mom wants to wear clothes and accrue profit' and then i watch a DS9 episode and its like 'Sisko got unstuck in time and Jake grew up a sad old man and killed himself in front of his timeshifted dad to send him back to before he got unstuck'

Look Outside on Nintendo Switch 2. Look Outside on Nintendo Switch. Look Outside on PlayStation 5. Look Outside on PlayStation 4. All on October 30. 🪟 👀

Finished CrossCode's main story last night (DLC up next). Extremely sick game that was made in a lab for and by the people who went 'what if isometric zelda required you to be doing DMC stance gameplay and also its puzzles were designed in a lab by geometry perverts'. Really really good

Nothing in the finale matched the absolute high of the midgame inflection point we hit but frankly it doesnt need to, because it was doing other sick shit instead that is absolutely bonkers. I'm not even rot13ing it but theres a lowkey insane thing it reveals in the final act lead-up that rules
